Adolescents, vulnerable populations have multiple sexual and reproductive health problems including unplanned pregnancies, unsafe abortion, and stigmatization on abortion. Stigma and its manifestations present key barriers to women's access to safe, legal and affordable abortions. Identifying social norms that stigmatize abortion and reducing abortion-related stigma are essential to make sure that all women are able to exercise their reproductive health rights. There are various cases of women who died because of unsafe abortion or committed suicide because of stigma in society. So in order to save the lives of women it is very necessary to reduce the negative stereotype and discrimination towards the women who do abortion on her own choice or due to various circumstances following the abortion policies. A cross section descriptive study was conducted to assess the knowledge and stigma regarding abortion among female adolescents of Ramdhuni Municipality. Sunsari district of Province 1 of Nepal using quantitative method. The study sit was selected purposively and the data related to site was collected from Municipality office. The study was conducted among 220 female adolescent students studying in class 11 and 12 of different schools of Ramdhuni Municipality. Random sampling was done to select the respondents for the study. Knowledge was assessed through structured questionnaire made after extensive literature review and through supervisor and guidance whereas stigma on abortion was assessed through the modified versions of SABAS scale i.e. ASBA scale used to understand the abortion stigma among female adolescent. Coding and entry of collected data were done in Epidata 3.1 and analyzed with IBM SPSS 20 version. Odds ratio and corresponding 95 percent confidence interval were used to test the strength of significance. Out of 220 respondents more than half (60.5%) had adequate knowledge similar to the level of magnitude of stigma on abortion was found to be 55% among respondents. Most of the respondent age group (16 -18) with mean ±SD age 17.47±0.808 years were participated. The majority were Hindu (98.6%), following Adhibasi/Janjati (52.7%) and Brahmin/kshetri (30.9%). More than half of the respondents educational status are class 11(51.8%) following majority in Educational stream (48.6%). Most of them were unmarried (98.2%). Most of the respondents 60.5% (n=133) had adequate knowledge on safe abortion whereas only 39.5% (n=87) had an inadequate level of knowledge. More than half 55% (n=121) of the respondents had a higher level of discrimination on stigmatizing magnitude on abortion whereas only 45% (n=99) had a low level of discrimination. Also, about 57.7% (n=127) had a higher level of negative stereotyping on stigmatizing attitudes, beliefs on abortion whereas only 42.3% (n=93) had a low level of negative stereotyping. About two third 60% (n=132) of the respondent had higher level of discrimination on exclusion and discrimination on stigmatizing magnitude on abortion whereas only 40% (n=88) had a low level of exclusion and discrimination. Similarly, half 53.2% (n=117) of the respondents had a higher level of fear of contagion whereas only 46.8% (n=103) had a low level of fear of contagion. The study concluded that more than half of the respondent possessed adequate knowledge on safe abortion. Younger respondent were found with adequate knowledge than older ones. Though adequacy in knowledge was found among respondent, the study revealed discrimination level was also higher in more than half of female adolescents. There still exist higher level of discrimination in spite of having knowledge on safe abortion. The municipality, school committee, local stakeholders should make plans and programs to reduce the level of stigma and discrimination towards abortion.
