Summary, etc. ABSTRACT: <br/><br/> Background: Dyslipidemia is one of the major risk factor of coronary artery diseases (CAD). To treat dyslipidemia, the physicians are prescribing various anti-lipidemic drugs, major being statin like atorvastatin, rosuvastatin, etc. The response of statins varies individually which may be due to various genetic polymorphisms. R219K polymorphism of ABCA1 gene is found to have role in the response of statin. <br/><br/> Objectives: The aim of this study was to evaluate the effect of R219K polymorphism of ABCA1 gene in the lipid lowering effect of statins in patients of dyslipidemia. <br/><br/>Research Method: A prospective study was conducted from December 2017 to November 2018 in cardiology OPD of MCVTC. A total of 88 blood samples were taken from patients with dyslipidemia after taking atorvastatin for 3months. Patients serum samples were analyzed for Lipid profile and compared with lipid value before medication and whole blood were analyzed for R219K SNPs using PCR-RFLP to evaluate its effect on lipid improvement following treatment. Data was then entered into excel sheet and analyzed in SPSS v20.0. <br/><br/> Results: R219K polymorphism caused significantly higher degree of reduction in value of serum TG (P=0.05), VLDL (P=0.05), TG/HDL ratio (P<0.05), and TC/HDL ratio (P<0.05) in atorvastatin users. However, it was not associated with reduction of serum TC (P=0.83), HDL-C (0.39) and LDL-C levels (P=0.89). Treatment of dyslipidemia was found to be comparatively better in patient with polymorphism than without polymorphism. <br/><br/> Conclusion: The KK genotype of R219K polymorphism of ABCA1 gene was associated with significantly greater degree of reduction of TG, VLDL, TG/HDL ratio, and TC/HDL ratio and subsequently with greater degree of increase in HDL-C following atorvastatin treatment in Nepalese patients with dyslipidemia. <br/><br/>Keywords: ATP Binding Cassette Transport protein A1 (ABCA1), R219K polymorphism, Lipid Profile, Statin, PCR-RFLP, Nepal.
