Summary, etc. | ABSTRACT: Objective- To study effectiveness of single dose Augmention in preventing post operative infective morbidity in elective major gynecological study. Mehods- A prospective and comparative study carried out in Maternity Hospital, Thapathali, where annually around 600 elective major gynecological surgery are done. During study period 149 elective gynecological were performed in the hospital. 96 women, who came for effective major gynecological, surgery, were enrolled in the study. Among which, 48 cases in group I were given 1.2 gm of Inj. Augmentin intra operatively, rest of the patient (48 cases) in group ii, were given 200 mg of Inj. Ciprofloxacin twice a day and Metronidazole 500 mg thrice a day for 24-48 hrs. post operatively. Then, they were given oral Ciprofloxacin 500 mg twice a day, & Metronidazole 400 mg thrice a day 5 more days. Febrile illness, urinary bacterial growth in post operatively catheterized patients, signs of wound infection and length of post operative hospital stay, were compared between groups. Results- Febrile morbidity were seen in one case each of both groups (2.08%). Both of them were seen in cases of abdominal surgery. The case of fever belonged to Augmentin group was associated with more blood loss (> 500 ml) and blood transfusion, where as other case did not have contributing factor. 5 cases (16.6%) out of 30 cases belonged to Augmentin group and 4 cases (13.8%) out of 29 cases belonged to Ciprofloxacin and Metronidazole group had urinary bacterial growth in their urine (p> 0.05). All the cases of urinary bacterial growth were seen only in vaginal surgical cases. 4 cases (10.8%) in Augmentin group and 5 cases (12.4%) in Ciprofloxacin group had induration in their abdominal and perineal wound. Significant difference in post operative hospital stay was not found between groups (p > 0.05). Infective events in patients were found associated with advancing age and more blood loss. Conclusion- Single dose (1.2 gm) Augmentin is as effective as multiple dose of ciprofloxacin in preventing infective morbidity in patients undergoing elective major gynecological surgery. |
