Correlation of plain x-ray findings with that of per-operative findings in CSOM(AA).

Online resources: Summary: ABSTRACT: Chronic Supurative Otitis Media ( Attico-Antral) is one of the commonest ear diseases in developing countries including Nepal. The disease is associated with bone eroding process and causes various intravranial as well as extracanial complications. It has been a routine practive by the otologist to ask for a plain x-ray of mastoia in a case of cases of CSOM (AA) in Nepal including Tribhuvan University Teaching Hospital (TUTH) Kathmandu. CT Scan & MRI are most important techniques for the evaluation of patients with CSOM in recent years. However in Nepal, because of cost factors and limited facilities we still have to depend on plain x-rays with CT & MRI being reserved for suspected cases of intracranial complications. A total of 50 patients who were diagnosed as CSOM(AA) were taken up for this study. All patients were asked to take x-ray mastids Twne's view and lateral oblique view in TUTH Radiology Department. X-ray findings were noted with the help of a consultant Radiologist. Modified Radical Mastoidectomy (MRM) was performed to all the patients by faculties of ENI & Head and Surgey Department. Findings of the operations were noted. Both, x-ray findings and operative findings were correlated. In this study, the radiological diagnosis of cholesteatoma was correct in 60% of cases. Regarding the extension of disease True positive co-relation was found more in key areas such as 60% in attic, 56% in aditus while antrum it was 38%. Dural plate was found intact both radiologically and per-operatively in 78% cases. Similarly sinus plate was found intact both radiologically and preoperatively in 88% cases. From this study it was concluded that from plain mastoid films substantial amount of information could be obtained when a well- trained professionals take them. Plain x-rays not only help in diagnosis, but also help surgeon to take precautions to restrict himself while drilling in the various key areas in the tympanomastoid segment.
